I. If you cannot say it, your patient cannot map it.
Most clinicians do not get stuck on technique first. They get stuck on comfort. The moment you normalize the vocabulary, the subjective history gets cleaner and the session moves.
II. “Before, during, after, or with ejaculation” is not trivia. It is your first differential.
Pain before sex can point you toward threat and arousal state. Pain during can point you toward movement, load, and local drivers. Pain after or with ejaculation can narrow the target fast. Many patients say “pain with sex” when they mean one specific moment.
III. Treat it like any other MSK problem. Track it, then test-retest.
A simple sex log separates noise from signal. It gives you baseline. It protects the patient from the 24–48 hour panic spiral. It also lets you connect the dots when the driver is not where they expected, like spine movement changing a genital symptom. Once you have data, your categories get simpler: spine, pelvic floor, nerve, tissue, plus the psychosocial layer that can keep the alarm on even after pain drops.
